I started teaching fourth grade midyear last year. I received a ton of resources from dear teacher friends, but as I was going through all of the back to school information... I found a ton of student information forms and goodies but there was something missing from each of them. It was time to just buckle down and make my own customized to my needs (or what I think are my needs so far!).
This is the packet that I plan on handing out at Meet the Teacher night. I am hoping that it asks for all the information I will need from parents to help us have a successful year. Let's keep our fingers crossed. I am also planning on making a few additional copies and keeping it on hand for any newbies I might get throughout the year!
The packet includes: Student information sheet, Getting to know the student/family sheet ,
Family/teacher communication preference, Parent Volunteer Interest form, Reading InterviewI went back and forth about including the reading interview form within this packet, but I decided this is something that I want filled out at Meet the Teacher night. Since we will be doing daily 5 this will give me a good glimpse at trying to get them started with books that they might enjoy for the first day.
